1. A single, 40 year old architect develops a fever and cough within 2 days of his hospital stay for an acute bout of diarrhoea. An FBC elicits a WBC of 15 x 109/L (Eosinophils 5 x 109/L), and a chest X-ray shows evidence of pulmonary oedema.
2. A 56 year old gentleman notices a painful, red patch around his cannula on his right hand following hospitalisation for pyelonephritis, treated with ciprofloxacin. Though he responds positively to treatment, he develops a cough that worsens steadily over the next 2 days, despite treatment with flucloxacillin. Shortly after, the patient acutely develops a fever and hypotension.
3. An 80 year old diabetic lady admitted for a UTI becomes feverish within 24 hours of arrival to hospital, with an associated cough and chest pain. Pus containing high levels of leucocytes and a high LDH is discovered on thoracentesis , confirming the diagnosis.
4. A 45 year old gentleman with a 45 pack year history of smoking develops a middle-ear infection and a productive cough. Symptoms improve with oral ciprofloxacin treatment.
5. A 22 year old woman presents with acute fever, a headache and neck pain. She also has a cough. A house officer is judged to stupidly ask for an X-ray prior to the registrar prescribing benzypenicillin. The X-ray, however, shows evidence of caseating granulomas. Her mother is extremely worried but is unable to stay at hospital with her any longer, as she has to see off her aunt from Calcutta who has been staying at home with them for the last year.
